We use this plugin and it does exactly what it says. With a little bit of work you can adjust design and templates to your needs and if you have any questions you can conntact support and ask for help.

Updated Review: Not working on mobile.I’m using this plugin on a site. I have installed an SMTP plugin for sending emails. Took one minute to set up the plugin. Very nice.
